This program helps patients with special medical needs. Case managers can help with transportation, support at home, supplies, and more. They work with doctors and local partners to set goals with members. Case managers follow members as they move between hospitals, nursing homes, and home.

Cascade Health Alliance has a program to help pregnant members. We work with many doctors to plan and manage your prenatal care. Our Case Managers can tell you about our reward program for you to do things that improve your health and the health of your baby. Some of those things are seeing your doctor, seeing a dentist, and taking your vitamins. If this is your first pregnancy, we offer you “Your Pregnancy Week-by-Week” to help you learn about what is happening in your body each week. You can also learn what is happening in your baby’s body. We care about your health and your baby.

We help all pregnant moms, including those with health problems. We can help you if you have problems with hypertension, diabetes in pregnancy, premature labor, abuse issues, homelessness, pre-eclampsia, drugs, and more.

Our goals for Klamath County pregnancies and babies are:

  • Better outcomes with lower costs
  • Early and regular prenatal care
  • Excellent prenatal, childbirth, and postnatal care and support
  • Coordination of prenatal services
  • Education for pregnant mothers

Cascade Health Alliance has supplies to help you breathe if your doctor thinks you need it. We can teach you how to use and care for your equipment. If you have certain breathing problems, we have special programs to help you cope.

We know how hard it is to quit smoking and using other tobacco products, and we want to help. When you’re ready to quit, we’re here to support you. We teach the American Lung Association’s “Freedom from Smoking” program. The classes are proven to help people quit. Your doctor can prescribe free nicotine replacement products.
